# Get an exam before you pay for a joint procedure

## Hot swelling, lost feeling, or weakness can't wait

Get quick medical help for a very warm joint with major swelling. Do the same for fever with strong joint soreness. Don't wait if the joint suddenly can't hold your weight. New weakness, lost feeling, or sudden loss of bowel or bladder control also needs urgent care.

## The first visit should end with clear choices

Ask who draws and spins your blood or marrow. For marrow care, a needle draws marrow from your pelvis. Fat-based care begins with liposuction to remove fat. Ask how much will be removed, how the area may feel later, and whom you'll call with a problem.

## Surgery may still be the right question

Heavy wear, a bent joint, or poor balance can mean a clinic procedure won't be enough. If surgery came up before, ask what may happen if you wait. Also ask how long recovery takes and what help you may need once home.

A meta-analysis of 9 Level-1 trials (671 patients) of adipose stromal vascular fraction and adipose-derived MSC injections for knee OA found SVF superior to saline or hyaluronic acid on pain and function at 3, 6 and 12 months - but INFERIOR to corticosteroid at 3 months, no different at 6, and only comparable or slightly superior at 12. Adipose-derived MSCs beat saline and conservative care but showed no significant difference versus HA. No serious adverse events were attributed to the injections themselves, though complications associated with the LIPOSUCTION step were observed.

The most widely used joint injection in medicine is itself associated with structural harm on repeat dosing - relevant context when a clinic frames a biologic as 'the alternative to steroid shots'.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.

A GRADE-rated systematic review and meta-analysis of 16 randomized trials (807 participants) found that MSC therapy for chronic knee OA pain PROBABLY RESULTS IN LITTLE TO NO DIFFERENCE in pain relief at 3-6 months (WMD -0.74 cm on a 10 cm VAS against a minimally important difference of 1.5 cm) or physical functioning (WMD 2.23 on the SF-36 100-point subscale against a 10-point MID), both moderate certainty; at 12 months pain was again probably little-to-no-different (WMD -0.73 cm). The measured effect is real but sits BELOW the threshold at which a patient would notice it.
